Congratulations on making a booking to see me as a new patient!
Here is what you need to do:
Before your appointment starts:
If you have blood test results for me to review, please email these to me in ONE document, the correct way up, no more than 24 hours before the consultation if at all possible. (Other types of test can be sent as a separate pdf.) I will review these during the consultation.
Please note that your virtual appointment will usually start a few minutes after the normal time. This allows me to read the information that you have emailed through, as well as your registration forms. I will join the call when I am ready (just like at your GP surgery; sometimes I am running late). On occasion there are technical issues where I cannot see that you are on the call. Please keep your screen refreshed (i.e. not in hibernation/sleep mode) and please keep an eye on your emails while waiting for me. If I cannot see you on the call I will email you to get things working again.
Before your appointment you need to complete a patient registration form and diet diary form. These can be found on your confirmation email. This is only necessary for new patients to the practice. Please allow at least 10 minutes to complete these.
Note that if you do not return your signed registration form we cannot treat you for liability reasons.
In our initial consultation we will go over your current diet and medications, and take a complete medical and family history. You will have time to tell your story and together we will formulate both short and long-term goals for your health and well-being.
Most patients require a 90 minute appointment as I see fairly complex cases that require a long initial time investment. For some simpler cases, I may advise one hour is sufficient. Your initial 90 minute appointment will cost $225. Payment for initial appointments will normally be via credit/debit card taken at the end of the consultation, though at our discretion we may request part payment before the appointment. If you wish to use direct deposit, please let me know as soon as you book so that you can pre-pay for your appointment time. Supplements will then be billed separately if necessary and mailed after payment is received, though most supplements are now provided direct by the supplier. (Existing patients may be permitted to pay by direct deposit after the appointment.)
If I feel that your diet is contributing to ill health we will take some time to formulate an eating plan that will assist your body to heal and remain healthy for life. Your diet is the foundation for any treatment plan and is essential for good health.
I will also look at your need for supplements, lifestyle changes and possibly other therapies and prescribe or refer for these as necessary. Once any nutritional needs are addressed we may have time in your initial appointment to discuss the spiritual aspects. If not, these will be addressed at your first follow up appointment.
Subsequent appointments will usually be 30 or 45 minutes long and cost $90-$120. For patients with a complicated case, or tests to review, an extended follow up is sometimes needed. Products are an additional cost, usually paid direct to the supplier when you place your online order.
You will receive a reminder email to confirm your appointment a few days beforehand. Responding to this email really helps me to plan and be ready for you. Please inform me as soon as possible if you need to cancel or move your appointment so that it can be offered to another patient (this can also be done via a cancellation link in your appointment confirmation email.)
